Rewatching the scene where Regina drinks the Barren Potion, I noticed something.
1. Regina is in severe pain.
2. Regina is crying.
3. Regina stares at the glass in what appears to be shock, and Cora tries to comfort her when she near collapses, you can hear Cora saying “No, No”.
Is it possible that when Regina drank the Barren Potion, she had a miscarriage because she didn’t know she was pregnant. And when Snow White took the barren potion King George gave her, I don’t remember Snow experiencing any sort of pain such as that.

I’m really hoping not. It was hard enough watching her drink that potion. To think she might have accidentally killed an unborn is a bit overwhelming…

I actually had the same thought. But i figured its probably not so. It’s been a year since Daniel, who could the father have even been? As to why Snow didn’t experience pain but regina did? Regina is a magic user and King George is not. Regina probably made the potion herself and it was probably more powerful and more painful. And plus it was probably pure while King George may have simply pour a little vial of the potion into, like, wine or something for Snow White so she didn’t notice.

Good explanation on Snow part…as for who the father could have been…I’m placing money on Graham.

I think it just hit her what she had done to herself…but I do like the theory that it was a slightly different potion than Snow got.

I think this is during Graham. Regina clearly appears to be in her evil Queen stage and Cora mentions Snow taking back the kingdom. Snow was still living at the castle when Regina hired Graham, and it was the attempt on her life that made her flee and caused Regina to drop the mask of loving step-mother and go full on evil queen.
All magic comes with a price!
