I think the last scene of the series will be in an episode titled Swan Song and will feature Emma celebrating her birthday surrounded by her family and loved ones so that it will bookend the pilot of Emma celebrating her birthday on her own.
Also Emma wished upon a star candle in the pilot which brings to mind this:

Will come to you
Emma always desired a family and to be loved. Hey presto! Guess what she got!
