If Neal and Cora both make it to Storybrook, maybe Cora will remove his heart, as she has been known to do. Neal could eventually end up in Oz, on a bid to get his heart back. If I remember the original Oz book properly, the tinman was originally a flesh and blood man who suffered a number of accidents requiring his limbs to be replaced by tin – something that could also happen to Neal.
