If the echo caves was about the characters revealing their deepest secrets, perhaps Emma’s secret wasn’t so much that she wished Neal had been dead but her revealing to Neal that she loves him, never stopped loving him, and always will love him.
Afterall, Emma spent the majority of season 2 pretending that she didn’t love him. When she said it to him over the portal, it was because she thought he was going to die and she didn’t have to deal with the consequences. Like when she called Snowing “mom and dad” she did it because she thought they were going to die but as soon the danger had past, she tried to take it back.
Her saying I love you to Neal in the echo cave was her having to reveal her secret and face the consequences of it.
