I’m certain it’s not about Cora or Regina being dead, at least not in the present.
There have only been 3 episodes in the entire series that had titles that wouldn’t refer to the flashbacks or FTL and that includes the pilot and season finale and I can’t see this episode being an exception to the rule.
I think having young Snow in it, clearly secures that it will be a story of Snow’s mom, even though I’d really like to explore Aurora’s story as well!

EDIT: correction, 4 episodes. I just realised The Cricket Game was referring to Storybrooke events which obviously kinda blows my theory but I’m still convinced the title does refer to FTL flashbacks and not SB.

    Here’s a theory of mine:

“The Queen is Dead” doesn’t refer to either Cora or Regina, but actually refers to the battle between Cora and Regina that ended with Regina believing the Queen, (Cora), to be dead. I doubt that they’d kill off either Regina or Cora, but my guess is that they will pretend that one of them is going to die all through the episode.

Or, yes, it might be referring to the battle between Cora and Regina, but in the present Storybrooke? The ‘mental’ battle that Cora would eventually fail, trying to convince her daughter onto her side? It could be a symbolic death, the Evil Queen – “the other woman” inside Regina – could be gone as she wants to redeem herself?

It would tie nicely if, at the same time, we would have some queen dying in FTL flashbacks?
