Theory: It’s Rumple’s dream, he wakes up and realizes that he needs Belle to keep him from going dark and goes to the hospital to win her back only to see that Regina got to her first.

I have to say, I’m glad this is Gold’s dream and not Bae’s or someone else’s. It shows he’s really struggling with this and he hasn’t decided what to do yet.

Plus, does this mean we get to see Gold wake up in bed? Sleeping shirtless? 😛

I don’t remember who said it but back when people were speculating that this was a dream someone said “what makes you think he wears pajamas….?”
