Charming would insist on repaying the debt until Rumple agrees just to get Charming to leave him alone. However, Neal hears that Charming owes Rumple a favour and will jump to the wrong conclusion: That Rumple went back on his agreement with Neal not to charge Charming for the cure thus Rumple and Bae’s relationship will take a step backwards.
I agree this is probably how it will play out, except that Charming is such a white knight, he’ll tell Nealfire that he insisted that he repay Rumple. And then Neal and Rumple can have another talk about trust. It’s going to be a long road. The reconciliation in 309 was GREAT but it doesn’t erase the past.
