Would I be way off target if I speculated that Regina might not know who the WWW is or what her vendetta is.
This is what I’m hoping for if they do end up being sisters. I think it could be interesting to see Regina have the tables turned on her.
In her situation with Snow, Snow knew that Regina hated her, but didn’t truly know why until that day when Regina told her that Daniel had died because of the secret she told. It wasn’t really Snow’s fault, she’d just been a child who was tricked into blabbing, but Regina still held it against her and it drove her desire to destroy her.
If WW hates Regina for something that isn’t really her fault, like jealousy over Regina having been raised with privilege, with their mother doting over her, (in her screwed up way), while WW had been given away and rejected and had to grow up alone, then Regina is essentially in the “Snow position” in that vendetta relationship.
And if they’re sisters, then in both the WW/Regina situation and the Regina/Snow situation, the instigator was Cora, which would tie it all together nicely.
Having Regina experience that other side of things could provide for some development of her relationship with Snow, perhaps lead to an actual apology, which would be part of the family continuing to grow closer.

A common theme that I’ve noticed might be occurring, in the second half, is many of our characters having the tables turned on them. Like characters in reversed roles.
Neal has now had to watch his son go to a new land without him, might help him see how his father felt losing him.
Emma, having now lived as Henry’s mother, might see how Regina felt seeing the threat of a new mother coming into his life.
Rumple, always having control over others through his dark powers, might come to know how it feels to have someone else controlling him.
Regina, who has demanded control over events and people in Storybrooke and her kingdom might now be subject to WWW’s (her sister’s) control. She might gain some empathy for those she controlled.
I’m sure there are many other examples of reversed roles playing out. Empathy might be a theme as each character lives out an adventure which has put them in someone else’s shoes.
