Maybe the outsider drove to where Belle is, saw her on the ground and got out to help then Hook quickly gets in and tries to hit Rumple but hits the man instead.
Because from what I could see in the canadian promo, the person who got hit by the car would have worse injuries than the person inside. So that might be why the outsider needs surgery and Hook just needs to be stitched up.
And I know that Hook has just arrived in SB and is getting used to his surroundings, but he already knows what a gun is, so maybe he know how to drive a car too, but he can’t exactly control it and thats why he hits a tree.I know this sounds a bit out there, but then again this show is unpredictable..
Hope this makes sense.
Other spoilers in the past (Wetpaint, etc) have already verified thatit's Ethan Embry's character (the outsider) who was driving the car.

ParticipantI like the idea of a spin-off. At least that way Rumbelle fans get what they hope for, and the writers get a fresh, brand new foundation to build on. I mean they can leave Storybrooke now, so why not?
This would also allow for them to build more on Hook without the restraints of a never-ending back-and-forth eye, for an eye thing. They’ve got to bury their axes at some point, y’all. It’d also allow them to build more on Rumple and Belle if they can get past the “Dark One” influence in some way. That too, is a brick wall they can only avoid for so long. They can let their foot off the gas, but the wall is still there, and so they’ve got to get him past that.

I received several reports about this topic, so I investigated it. I think this is still a legitimate theory based on some spoilers, but it could still be false and many people wish for either side.

It sounds like this theory had merit. That makes me very unhappy, and honestly just ruins my whole day. I know it’s just a tv show, but this seems like a bait and switch on the people who hold it up (the viewers) and so if Aesop’s right, then yeah it ruins that for me too.
